Coronavirus and Med3 letter to patients

 

You do not require a doctor’s sickness certificate for any illness lasting seven days or less. Your employer may, however, may require you to complete a self-certification form SC1 which is available to download via the link below.

For any illness lasting longer than seven days or more you will need to see the doctor for him or her to assess the medical condition and to issue a sickness certificate if necessary.

For on going Med3 certificates you do not need a GP appointment. Please contact the surgery on the date the sick notes runs out using e-Consult. Please do not contact us prior to the expiry date as we cannot post date a Med3 certificate. This will then be forwarded to the appropriate GP to process the request. Please note it can take up to 72 hours for us to process a Med3 request.

For return to work notes PLEASE NOTE THESE CAN ONLY BE ISSUED ON THE DATE OF THE RETURN TO WORK.

Some employers insist on a Doctors Sickness Certificate for the first seven days, if this is the case then a private sick note can be issued for a fee of £10.00.
