Med3 Fit Notes

You do not require a doctor’s sickness certificate for any illness lasting seven days or less. Your employer may, however, may require you to complete a self-certification form SC1 which is available to download via the link below.

For any illness lasting longer than seven days or more you will need to see the doctor for him or her to assess the medical condition and to issue a sickness certificate if necessary.

For on going Med3 certificates you do not need a GP appointment. Please contact the surgery on the date the sick notes runs out using e-Consult. Please do not contact us prior to the expiry date as we cannot post date a Med3 certificate. This will then be forwarded to the appropriate GP to process the request. Please note it can take up to 72 hours for us to process a Med3 request.

For return to work notes PLEASE NOTE THESE CAN ONLY BE ISSUED ON THE DATE OF THE RETURN TO WORK.

Some employers insist on a Doctors Sickness Certificate for the first seven days, if this is the case then a private sick note can be issued for a fee of £10.00.

  • We provide a text messaging service to help us to contact you for recalls, appointments, reminders. Do you feel that this has helped in receiving information in a timely manner?
  • When you have contacted us by the telephone, have you been satisfied with the advice you have been given when you have phoned to make an appointment?
  • We have introduced EConsult in October 2019. EConsult is an on-line consultation software accessed through the Practice website. Do you feel that it is becoming easier to make contact and to get appropriate advice?

  • From March 2017, repeat prescriptions can no longer be ordered by the community Pharmacist. Orders can be taken via email, post, or box in reception. Do you feel supported in managing your medication?
