  • Dear Patient Our records still show that you have not attended for a repeat cervical smear despite having been sent reminders. We strongly recommend that you continue to have cervical smears. Please contact the Surgery at the above number so that we can arrange a convenient date and time. Should you decide that you don’t want to have a smear at this time, please sign below and submit to the Surgery and your name will be suspended from the recall list. Should you at anytime wish to have a smear, please just make an appointment at the Surgery.
