Sick notes, fit notes and self certification

You may need a sick or ‘fit’ note if you have been off work for 7 days or more.

You can request a sick note by completing a systmconnect form.

If you’ve been off for 7 days or fewer

If you’ve been off work, sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill.

Instead, they can ask you to confirm that you’ve been ill by completing a self-certification form.
